Objective To investigate whether the presence of kidney stones increase the risk of end stage renal disease (ESRD) or other adverse renal outcomes.
Design A registry cohort study using validated algorithms based on claims and facility utilisation data. Median follow-up of 11 years.
Setting Alberta, Canada, between 1997 and 2009.
Participants 3 089 194 adult patients without ESRD at baseline or a history of pyelonephritis. Of these, 1 954 836 had outpatient serum creatinine measurements and were included in analyses of chronic kidney disease and doubling of serum creatinine level.
Exposure One or more kidney stones during follow-up.
Main outcome measures Incident ESRD, development of stage 3b-5 chronic kidney disease (estimated glomerular filtration rate <45 mL/min/1.73 m2), and sustained doubling of serum creatinine concentration from baseline. Results 23 706 (0.8%) patients had at least one kidney stone, 5333 (0.2%) developed ESRD, 68 525 (4%) developed stage 3b-5 chronic kidney disease, and 6581 (0.3%) experienced sustained doubling of serum creatinine. Overall, one or more stone episodes during follow-up was associated with increased risk of ESRD (adjusted hazard ratio 2.16 (95% CI 1.79 to 2.62)), new stage 3b-5 chronic kidney disease (hazard ratio 1.74 (1.61 to 1.88)), and doubling of serum creatinine (hazard ratio 1.94 (1.56 to 2.43)), all compared with those without kidney stones during follow-up. The excess risk of adverse outcomes associated with at least one episode of stones seemed greater in women than in men, and in people aged <50 years than in those aged ≥50. However, the risks of all three adverse outcomes in those with at least one episode of stones were significantly higher than in those without stones in both sexes and age strata. The absolute increase in the rate of adverse renal outcomes associated with stones was small: the unadjusted rate of ESRD was 2.48 per million person days in people with one or more episodes of stones versus 0.52 per million person days in people without stones. Conclusion Even a single kidney stone episode during follow-up was associated with a significant increase in the likelihood of adverse renal outcomes including ESRD. There is increasing evidence (Sox, 2002; Papadakis et al., 2004; ABIM, 2003) that unsatisfactory performance in practice is more likely to be due to unprofessional behaviour, rather than knowledge or clinical skills. At the University of Sheffield, such an opportunity for reflection on professional behaviours arose in the Intensive Clinical Experience (ICE) within the first year of the course.

We validated our professional behaviours outcomes by mapping them against the principles of Duties of a Doctor (GMC, 2001) (Figure 1).

Within the postgraduate arena, the tenets of Good Medical Practice have been used to create a multi-source feedback tool to assess a range of generic skills, which cover aspects of professionalism in the workplace (Archer et al., 2005). We wished to use the portfolio approach with the purpose of marshalling evidence about the progress of students towards the specific professionalism outcomes of our course (Challis, 1999). 

[Method] 

This study aimed to discover the educational impact, validity, and feasibility of the critical incident as an assessment method for a class of students undertaking guided reflection in the context of their first exposure to health and social care professionals at work.

The ‘guided reflection’ method (Johns, 1994; Wilkinson, 1999) was adapted to prepare students to engage in reflective practice (Figure 2). We defined a ‘critical incident’ (Flanaghan, 1953) as any event that challenged them within the context of Duties of a Doctor.

[Outcome measures]

We used two outcome measures to provide data for our study. These were: (1) The quality of student reflections. (2) Student evaluation of ICE (the Intensive Clinical Experience).

[Qualitative data analysis]

A content analysis using a constant comparative approach was used to provide a basis from which a conceptual framework could emerge in relation to our research questions. Validity was assured by iterative consideration of the emerging explanations for the data. The number of times each code was evidenced in the full data set (Table 1) is given.

[Results]

The reflections were analysed and 40 codes assigned. The codes were merged into 11 sub-themes (Table 2) and from those, five themes were identified: communication; professionalism; team working; organisation of care; and student learning issues. Professionalism encompassed the greatest number of sub-themes and included reflections on the behaviour, professionalism and the quality of care given by all three professional groups. The most frequent code was ‘dignity, autonomy and patients’beliefs'(n=30).

[Discussion]

Guided reflection has a valuable educational impact on our students in the exploration of professionalism in a real work-based multi-professional setting. Reflecting on critical incidents encouraged students to understand and analyse professionalism, and recognize what it means to be a professional in the context of Duties of a Doctor.

高齢者の糖尿病治療戦略

 高齢者のDM治療ではエビデンスの弱さや最近のRCTから、いつもその目標値に悩むことが多かった。今回ADAとアメリカ老年医学会からのコンセンサスレポートがOnlineで出版され、今後の治療戦略に練り直しが必要と感じため共有する。

- 要約 -
 2012年2月にADAは65歳以上の糖尿病治療のコンセンサス作成会議を行い、以下の項目について各分野の専門家
からのプレゼンを受けてレポートを作成した。

【糖尿病高齢者の疫学と病態生理】
 米国の高齢者の22-33%が糖尿病。過体重や肥満との関連が指摘されている。高齢発症と中年発症の2タイプが
存在し、網膜症は中年発症タイプに多いが、CVD発症や神経症については発症年齢による差がない。
 下肢切断、心筋梗塞、失明などの合併症は65-74歳の群よりも75歳以上の群に多い。

【予防と治療】
 高齢そのものは前糖尿病/糖尿病のリスクである。生活習慣や各種治療で予防や発症遅延が可能となる。

 血糖コントロールについて
 ・UKPDS後の3つのトライアル(ACCORD、ADVANCE、VADT)が登場し、CVDを予防するための
  血糖コントロールの割が明らかになったものの、高齢者においての厳密な治療の功罪について不確実性が増した。
 ・最近の日本での65歳以上のトライアルでも多様な介入群と標準治療群での差を認めず。
 ・今までのRCTでは高齢者を含まないことが多く、血糖コントロールと高齢糖尿病患者との関係を観察することで
  示唆を得られるだろう。英国の50歳以上の観察研究では、単剤治療群と追加(内服/インスリン)治療群において
  A1cと死亡率の関係がU字型であり、もっとも死亡率が少ないのがA1c7.5%であった。

 脂質管理について
 ・高齢糖尿病患者についての脂質管理の大規模トライアルは存在せず、心疾患イベント抑制のエビデンスは
  高齢者群ではられたものにとどまっている。

 血圧管理について
 ・多数のトライアルにおいて降圧治療は心血管イベントを抑制するが、高齢糖尿病患者においての効果は
  限定的である。
  収縮期血圧170以上の群を150台に下げることの効果は指摘されており、140/80以下に降圧することの効果は
  選択されたトライアルで示されている。
 ・その他のコホート研究においては収縮期血圧を140以下に積極的に下げる利点はなく、高齢者においては
  拡張期血圧の低下が亡率上昇のリスク因子になるかもしれない。

【最近の高齢糖尿病患者への治療ガイドライン】
 ・身体機能、認知機能に障害のない高齢者は若年者と同様の治療目標となる
 ・高齢者の血糖管理目標は個々に緩めて決定されるが、高血糖による有害事象は避けなければいけない。
 ・その他の心血管イベントのリスク因子はそれぞれ治療され管理されなければいけないが、
  効果のある時間軸や患者の個別性への考慮が必要となる。
 ・合併症の評価も個別的なプランが必要となりつつ、機能障害への影響は注意しないといけない。
 ・2010年の退役軍人病院のガイドラインでは、以下の3つのカテゴリーに分けている
  *微小血管合併症がないかわずかなで生命予後が少なくとも10~15年の群はA1c7%以下
  *10年以上の糖尿病罹患期間がある、もしくは併存症の状態からも必要とされる群はA1c8%以下
  *進行した微小血管合併症があり重大な併存症があり、生命予後が5年以下と予想される群はA1c8~9%
 ・欧州の高齢糖尿病治療のガイドラインでも”治療決定に際し、低血糖や自己管理能力、その他の病的な
  異常の有無、知状態や生命予後を考慮して治療の功罪を検討すべき”とある。

【高齢患者への治療の個別化】
 ・併存症や虚弱性についての評価(移動障害、転倒骨折リスク、多剤管理、鬱状態、視力聴力障害が考慮される)
  のみならず、齢者独自の栄養療法、自己管理教育や指導の個別ニーズへの配慮、身体活動性と健康維持や
  年齢を考慮した物治療管理が求められる。
 ・メトフォルミンは治療の第一選択であるが腎障害で減量が必要となる。SU系は低コストであるが血糖
  リスクがある。
  αGIは高齢者には理論的には魅力があるが消化管への影響から忍容性が低くなる、DPP4は食後高血糖低下に
  役立ち忍容性もよいがコストの面で制限がある。
  インスリン治療は適応を選択して利用するとすぐれた治療になるが低血糖リスクが高く75歳以上においては
  限られた情報しかない。
 ・生命予後が治療目標に影響する中心概念となる。
 ・意思決定においては患者の目標と医学的な目標の間の中で一致点を探すコミュニケーションが必要となる。
 ・療養環境が自宅か否かも考慮され、例えば施設ケアの患者の治療は働くスタッフの負担を考える必要がある。

【高齢患者への推奨治療】
 3分類のフレームワークは治療効果の時間軸と生命予後とのバランスを考慮した一つの試みである
 ・〔患者の特徴と健康状態〕を3群に分け、それぞれ〔根拠となる生命予後〕
  〔理にかなった治療目標(低血糖などが無ければ個別にそれ以下の目標を設定できる)〕
  〔空腹時もしくは食前の血糖レベル〕〔就寝前血糖〕〔降圧目標〕〔脂質管理〕を置いた。
 ・健康群(併存症がほとんどなく認知/身体機能に問題なし)では、A1c7.5以下、空腹時血糖90-130mg/dL、
  血圧140/80以下、脂質は問題なければスタチン使用
 ・中間複雑群(併存症があり、IADLに2つ以上の障害or軽度~中等度の認知機能障害)ではA1c8.0以下、
  空腹に90-150mg/dL、血圧140/80以下、脂質は問題なければスタチン使用
 ・高度複雑群(健康状態不良群、長期や終末期の健康問題が複数or高度の認知機能障害orADLが2つ以上障害)では
  A1c8.5%以下、空腹時100-180mg/dL、血圧150/90以下、脂質はスタチンの利点が上回れば使用
  *併存症とは薬物療法や生活の制限が求められる状態で、リウマチやがん、心不全、うつ病、肺気腫、転倒、
   高血圧、排尿障害、CKD(stage3以上)、心筋梗塞および脳卒中が含まれる。(Table1参照)
  更なる推奨はTable2、今後高齢糖尿病患者に必要なリサーチクエスチョンはTable3に記載された。

Definition of “Access to care”: “The ability to obtain health services when needed” (Bodenheimer TS etal. Understanding Health Policy. East Norwalk, CT: Appleton & Lange, 1995).
Of the five principles of family medicine, access to care has been associated most strongly with positive health outcomes.
 Access to care is not the same thing as access to health; Access to medical care is a less powerful determinant of population health than sanitation, nutrition, and socioeconomic class.
  Evidence that access to care is an important determinant of health is extensive and can be classified as follows.

1. Access to certain types of preventive services is associated unequivocally with decreased morbidity and mortality(ex. immunizations, prenatal care).
2. Access to care that allows management of certain chronic medical conditions prolongs life and minimizes complications (ex. hypertension, diabetes).
3. Access to primary care has been shown to decrease hospitalization rate and probably decreases overall health care costs(Casanova C, Starfield B. Int J Health Sev 25:283-294,1995. Bindman AB et al. JAMA 274:305-311, 1995.).
4. Substantial data establish that access to care is an important determinant of patient satisfaction with care.

【Access to Acute Care】

The ability of the patient or family to obtain access to the physician or physician’s office staff by telephone has become an important quality-of-care measure in Amerian medicine.

【Access to Preventive Care】

In U.S., the HMO is holding family physicians accountable for preventive services of habitants. The HMO believes that its expectations are reasonable, since it is paying family physicians a monthly capitation fee to assume this responsibility. A system of EHR will make it easier to monitor which patients have and have not received a particular preventive service or set of preventive services.

【Access to Chronic Care】

Family physicians believe that frequent visits with chronically ill patients improve doctor/patient compliance and facilitate a more rich and complex relationship between the patient, physician, and family.

【Access to Specialty and Hospital Care】

Highly technical and specialized care can harm patients as well as help them. Thus, assuring and managing access to specialty care has become a central part of family practice.

【Managing Underaccess to Care】

1.Overcoming Financial Barriers
inancial barriers to access in a given practice can best be overcome by specifically discussing these issues, establishing a written policy for the practice, and communicating and enforcing the policy in a caring and humane manner.
2. Overcoming Geographic Barriers
Telecommunications technology can be used to extend the physician’s service range( ex. Psychiatrist see patients with this technology in Rebun island,)
3. Overcoming Cultural Barriers
Family physician should evaluate the language needs of the practice and should establish a plan to care for patients when language barriers are present. The practice should also evaluate the cultural and religious needs of the community as an important step to assure access to care.
4. Overcoming Family Barriers
Decisions are often made by family health decision makers. Family physicians should include an assessment of this process in all formal family assessments and should also assess family health decision making for patients with chronic illness.
5. Overcoming Health System Barriers
If it is difficult for patients to access care on weekdays during the day, then family physicians should open their offices in the evenings and on weekends.
6. Overcoming Educational Barriers
The protocol for scheduling routine appointments, obtaining prescription refills, or getting health advice by telephone should also be provided to patients in writing when they enroll in the practice and reinforced periodically.

Overaccess to care

Overuse of care happens when there is a mismatch between illness and illness behavior.

子供はCTによる被曝によって、どのような影響を受けるか?

 当診療所ではCT検査が可能である。年に数人は小児のCT検査を施行することがある。今回CT検査から受ける放射線により、白血病、脳腫瘍の発生に対する影響を評価するコホート研究が発表されたため、紹介したい。

- 要約 -
【背景】
 CTは臨床的に非常に有用である一方で、電離放射線に関連した潜在的な発癌リスクが存在する。しかも大人よりも子供は放射線への感受性が強い。今回子供と若年成人のコホートでCTスキャン後の白血病と脳腫瘍の存在を評価したい。今までCT施行後の患者における発癌リスクの直接的な研究はなかった(新奇性)。
【方法】
 後ろ向きコホート研究で、1985年から2002年にイングランド、ウェールズ、スコットランドのNHSにおいてCT検査を受けた、当時22歳以下の方で、過去に癌の診断を受けていない患者を対象とした。1985年1月1日から2008年12月31日までにNHS名簿から癌発現率、死亡、フォローアップ不能のデータを採取した。CTスキャンごとに脳と赤色骨髄に吸収された線量(mGy)を計算し、ポワソン相対危険モデルで白血病と脳腫瘍の過剰発現を評価した。癌を疑った際の診断目的に施行されたCTの算入を避けるため、初回のCT検査から白血病の診断に至った期間が2年間、脳腫瘍であれば5年間以下の者は除外した。
【結果】
 追跡期間中に178,604人の患者のうち74人が白血病と診断され、176,587人の患者のうち135人が脳腫瘍の診断に至った。CTスキャンと白血病に関連のある放射線濃度はERR/mGyは0.036(95%CI0.005-0.120;P=0.0097) であり、脳腫瘍では過剰相対リスク0.023(0.010-0.049;P<0.0001)であった。5mGy以下の線量を受けた患者と比較して、少なくとも累積30mGy(平均51.13mGy)の被曝をした患者では白血病の相対危険度は3.18(95%CI1.46-6.94)であった。また累積50-74mGy(平均60.42mGy)の被曝をした患者では脳腫瘍の相対危険度は2.82(1.33-6.03)であった。 【解釈】  小児が約50mGy(脳CT5~10回)の累積被曝をした場合、白血病のリスクが約3倍で、脳腫瘍においては約60mGy(脳CT2~3回)で3倍になる。これらの癌はまれなものである故、累積寄与危険度は低い。10歳以下の小児で初回のCT検査から10年間において考えてみると、10,000回の頭部CT検査で1人の白血病、1人の脳腫瘍が生じる計算となる。臨床的な利点が相対危険度を上回ることは確かだが、CT検査の放射線濃度は可能な限り低く保つべきあり、妥当性があるのであれば電離放射線を生じない他の方法を考慮すべきである。 参考 相対リスク(RR)と過剰相対リスク(ERR)       一般的な疫学研究において2つ以上の集団でリスクを比べる時、非曝露群と比べて曝露群が「何倍」のリスクがあるのかをみるのが相対リスク(RR: relative risk)で、1以上であれば曝露群のリスクが高く、1以下であれば曝露群のリスクが低い。例えば、被曝線量がゼロの集団における疾患Aの発生リスクが10万人あたり2人で、被曝線量が1 Svの集団では10万人あたり5人だった場合、単純なRRは5/2=2.5倍となる。しかし、放射線被ばくのリスク評価では、放射線を浴びることによって単位線量当たりどのくらい過剰にリスクが上昇したのかをみる過剰相対リスク(ERR: Excess relative risk / 1Sv)という指標が用いられることが多い。上記の例では、単純なERRは(5-2)/2=1.5となり、被曝線量1 Sv浴びると1.5倍過剰に発生リスクが上昇することを意味する。しかし、実際の被曝者集団における解析では、被ばく線量、被ばく時年齢、被ばくからの経過時間、現在の年齢、性別などの、様々な因子を考慮して解析されている点に注意が必要である。 開催日:2012年8月29日
